Using the form builder inside OU should not be used to collect level 1 or 2 data. CSU data classifications link opens in new window

Collecting user data requires you go through ISO Policies and Standards Opens in new window and use SSL.
Required Elments in each form:
  • fieldsets
  • label with for attribute
  • input boxes
  • description information (alt/title) and aria attributes link opens in new window
  • submit button and/or reset button
By default all results are emailed to one or more users. No database or stored results are available as an option at this time.
